Subject:        Shakespear's(?) Titles

My question is: Is there any evidence that the titles of Shakespeare's
plays are attributable to Shakespeare himself, either as the author or
having approved of them? or are they attributable to others (printers,
publishers, etc.)? or do we not know?

In connection with this: is there any one or more than one good
treatments of this question, preferably not over-long?
