Todd Pettigrew asks

Kevin Quarmby does. He played the cream-faced loon in Peter O'Toole's
1980 Macbeth. He recalls the production in his article "A Twenty-fifth
Anniversary Study of Rehearsal and Performance Practice in the 1980
Royal Court Hamlet and the Old Vic Macbeth: An Actor's View", which
appeared in volume 1 (2005) of the journal Shakespeare at pages 174-187
(DOI 10.1080/17450910500399323)*.
